  • Pre-session intake forms must be filled out 24 hours prior to your appointment time. This allows me to analyze your experiences beforehand, ensuring you get the most from our time together.

Part 2 Intake
45-60 minutes

  • If needed, wraps up the intake, clarifies history, and addresses questions

  • Helps ensure a complete assessment, especially for complex or layered symptoms

  • Some may only need the first session; others benefit from a two-part intake

Your intake is focused on understanding your story. This includes exploring mood symptoms, trauma history, patterns of thought and behavior, and experiences that may have been misunderstood or misdiagnosed. You choose how deep we go.

This process is designed to capture high-masking, complex,

or previously misunderstood presentations.

Structure & Timeline:

Pre-Appointment

Before we meet, you’ll complete an intake packet that includes your medical, personal, and psychiatric history. You’ll have space to share as much or as little detail as feels comfortable. ​

This information helps me begin to understand your patterns and experiences before our first session. ​

We will talk through everything together, so there is no pressure to answer perfectly. The goal is simply to start building a thoughtful, comprehensive picture of you.

Comprehensive Psychiatric Intake

(Appointment #1)

A thorough review of medical, psychiatric, and

developmental history

Insurance-billable or self-pay ($275), 60-75 minutes

Insurance coverage may reduce or eliminate your out-of-pocket cost.

  • Exploration of current concerns, patterns, goals, & review of intake questionnaires 

  • Comprehensive evaluation for mental health conditions & analysis of symptoms

  • Clarification of symptom overlap and possible diagnostic pathways

  • Determines whether a formal neurodivergent evaluation is appropriate and, if so, which pathway will be most appropriate

  • There is no obligation to proceed with further evaluation

  • Note: a formal ADHD or autism diagnosis is not made during this visit

Diagnostic

Evaluation

(Appointment #2)

Based on the intake, we will choose the most appropriate evaluation path (see the two options below).

 

Prior to the evaluation, I will send several targeted assessment measures related to ADHD, autism, executive function, and other neurodivergent traits.

Because ADHD and autism frequently co-occur (more than half of autistics have ADHD!), I also assess for ADHD when autism is suspected, along with other common overlapping neurodivergent traits.

Option 1: Comprehensive Neurodivergent AuDHD Evaluation

(Autism & ADHD)

A structured comprehensive evaluation assessing autism, ADHD, and related neurodivergent traits. Designed for individuals seeking a comprehensive understanding of both autism and ADHD, including how these patterns interact.

Self-Pay only, $600, 2-3 hours

  • A structured clinical interview, utilizing the Monteiro Interview Guidelines for Diagnosing the Autism Spectrum (MIGDAS), and the Diagnostic Interview for ADHD in adults (DIVA)

  • Screening tools & questionnaires as completed prior to the session, behavioral observations, & collateral input when helpful (family, partner, teachers)

  • Includes a Diagnostic Assessment Report summarizing diagnostic impressions, clinical rationale, key findings, functional impacts, and personalized recommendations

  • Optional Comprehensive Neurodivergent Profile Report (+$125): an expanded personalized report that goes beyond diagnostic findings to provide an in-depth analysis of cognitive, social, sensory, emotional, executive functioning, masking, and self-regulation patterns. See more below.

A focused evaluation for one concern:

AUTISM EVALUATION

Self-Pay only, $400, 1.5-2 hours

  • An evaluation for individuals seeking an autism diagnosis. Includes developmental/clinical assessment, structured interview (MIGDAS), targeted measures, diagnostic formulation, and Diagnostic Assessment Report.

  • Screening tools & questionnaires as completed prior to the session, behavioral observations, & collateral input when helpful (family, partner, teachers)

  • Optional Comprehensive Neurodivergent Profile Report (+$125), see more below

ADHD EVALUATION

Self-Pay only, $300, 1-2 hours

  • An evaluation for individuals seeking a formal ADHD diagnosis and diagnostic documentation. Includes targeted assessment measures, clinical interview (DIVA), behavioral observations, diagnostic formulation, and Diagnostic Assessment Report.

  • Optional Comprehensive Neurodivergent Profile Report (+$125), see more below

  • Note: If you are receiving ongoing psychiatric care with me and prefer to explore ADHD symptoms as part of our ongoing treatment, a separate formal ADHD evaluation is not required. ADHD can be assessed clinically over the course of treatment when appropriate. This option does not include a separate diagnostic report or formal diagnostic documentation.

Results & Report

(Appointment Optional)

The standard Diagnostic Assessment Report summarizes diagnostic impressions, clinical rationale, key findings, functional impacts, and personalized recommendations. This report provides clear documentation of assessment results and can be used to support treatment planning, self-understanding, and accommodation requests when appropriate. It is included in the evaluation cost.

The optional Comprehensive Neurodivergent Profile Report (+$125) is an expanded personalized report that goes beyond diagnostic findings to provide an in-depth analysis of cognitive, social, sensory, emotional, executive functioning, masking, and self-regulation patterns. Includes individualized strengths, challenges, insights, and tailored recommendations designed to support deeper self-understanding and personal growth. Can range from 8 to 15 pages depending on the case.

 

If you would like to schedule a follow-up to review these results and discuss a treatment plan and next steps, you may. I encourage you to ask as many questions as you would like. You deserve to have full clarity!

The total cost above reflects the maximum out-of-pocket cost if insurance is not used for the initial intake (appointment #1). In many cases, the first appointment may be partially or fully covered by insurance, reducing your total cost.


The second evaluation appointment is self-pay due to the extended time and depth involved, inside and outside of the appointments.

medication support, that’s okay too! We will tailor your care to your needs.
